Distance:

To use the chart . . . . "From" is at the top, "To" is along the side.   This is important only because of examples like TPM, where Coruscant is farther from Naboo than Tatooine, but we don't know which one Tatooine is closer to because we don't know which direction it was from Naboo.

Other Worlds:  Bestine and Commenor are mentioned in the ANH novelization, and Ord Mantell in the TESB one, but there is no way to get useful information about them.

TESB Oddity:  Hoth, Anoat, and Bespin should all be absurdly close together, given the suggestion of sublight travel between them in TESB.  The Falcon piggy-backed on the Avenger from Hoth to "near Anoat", and it was only after Anoat that the Avenger went to hyperspace, with the Falcon dropping off beforehand.  Then the Falcon, with non-functional hyperdrive, proceeded from near Anoat to Bespin.   It's possible that Anoat and Hoth were so close as to be nearly colliding, which would explain the extensive and violent asteroids.   The Falcon would still have needed some sort of back-up hyperdrive for the Bespin trip, however, unless they were aboard the Falcon for years.  In any case, I've marked all three systems as close to one another.

Ald-Tat.  As judged by Han's comment to Jabba in the ANH novelization:  "But I've got a charter now and I can pay you back, plus a little extra. I just need some more time. I can give you a thousand on account, the rest in three weeks."   Distance value derived from known hyperdrive velocities (see E-S or G-T).  The Falcon's great speed might make the distance greater, though how much greater is unknown.

Ald-Yav.  As judged by Vader's "this will be a day long remembered" comment, after killing Obi-Wan while the DS was at the remains of Alderaan, and then preparing for the destruction of Yavin.  Distances obtained from known canon hyperdrive velocities (see E-S or G-T).   I would expect it to be on the lower end, given the sheer bulk of the Death Star and thus the probably-low hyperdrive speed.

Dag-Hot.   "Not far" as per Luke's comment to R2 after departing Hoth, as per the TESB novelization.   Though an X-Wing's maximum hyperdrive range is not known, RoTJ shows us that it must be somewhere above about 200 light-years.

End-Sul.  As judged by the comment on the distance from Sullust to Endor in the RoTJ novelization.  Note that in the book, the Rebel fleet actually left from Sullust orbit, but this was not so in the film.  More on this can be found here.

Geo-Cor.  More than Geonosis to Naboo, as per the range limitations of Obi-Wan's transmitter in AoTC.

Geo-Nab.  Less than Geonosis to Coruscant, as per the range limitations of Obi-Wan's transmitter in AoTC.

Nab-Cor. More than Naboo-Tatooine, as per the damaged and leaking hyperdrive preventing Queen Amidala's ship from reaching Coruscant in TPM.
